Office Policies

Scheduling Appointments:

Out of necessity, our physicians must work by appointment. Appointments can be made by calling our office.
OUR PHONE HOURS:
Monday & Tuesday - 8:15 a.m. to 5:00 p.m
Wednesday - 8:15 a.m. to 1:00 p.m
Thursday & Friday - 8:15 a.m. to 3:00 p.m.

For emergencies, call Memorial Hospital and Healthcare Center at (812) 996-2345 and a doctor will be contacted for you.

If you need need to cancel or reschedule your appointment, we ask that you notify us 24 hours in advance.
+ Failure to do so might result in a $25 Missed Appointment Fee.

Financial:

We accept most medical insurance plans. It is the patient’s responsibility to call your insurance company to make sure we accept your particular insurance. We accept assignment as Medicare providers.

All co-payments, deductibles and charges for non-covered services must be paid at the time of your office visit. We accept cash, checks, credit and debit cards. If necessary, payment plans for patients can be arranged in advance. We will file your claim to your insurance company. A copy of your insurance card and driver's license will be made for our records. Please bring your current insurance card to every appointment.

During your office visit exam, it may be necessary for the doctor to perform a refraction, measuring your best corrected vision for glasses. This is considered a separate exam from the office visit exam. Most insurance companies consider this a non-covered service. If this procedure is performed and you would like a glasses prescription written out for new glasses, a payment of $40 must be paid at the time of service.

ALL BILLING WILL NOW BE PROCESSED THROUGH DR. ENGLERT’S MAIN JASPER OFFICE, SOUTHERN INDIANA EYE ASSOCIATES. THE JASPER OFFICE ONLY ACCEPTS MEDICAL INSURANCE SO VISION PLANS WILL NO LONGER BE ACCEPTED. THERE ARE SOME INSURANCE PLANS THAT WAS ACCEPTED AT THE EYE GROUP THAT SOUTHERN INDIANA EYE ASSOCIATES DOES NOT ACCEPT. PLEASE CONTACT YOUR INSURANCE COMPANY TO MAKE SURE WE ACCEPT YOUR MEDICAL INSURANCE.

PRESCRIPTION REFILLS:
We ask that you call your pharmacy to request any refills needed for your medication. They will fax us a refill request.
